We are currently accepting new patients, to register as a patient please check you are within our boundary (please see below). If yes you will need to complete a patient registration form, these need to be collected from our reception team, they cannot be emailed (which is accessible via an intercom system during the COVID-19 pandemic).
Patients who live in the area and who wish to register with the Practice are asked to complete our Health Questionnaire at the time of registration. A health professional will check the information provided and, where necessary, patients will be invited to make an appointment for an appropriate check.
Please see below an approximate outline of our boundary, however, if you feel you may live outside of this, please ring to enquire before registering. If you wish to register at the surgery visit our reception desk.
To register with the practice you need to complete the Cadbury Heath Healthcare registration Form below. Before your registration can be accepted you will need to present 2 forms of identification to reception. One to confirm your personal identity (e.g. passport) and one to confirm your address (e.g. utility bill). We also require you to bring in a completed GMS1 Registration Form, which you can obtain from our reception team.
